Hospitals, social workers, case managers, placement agencies and families may initiate a referral to either Joy in Caring home.
Start with basic information
Use the general referral inquiry to connect with Joy in Caring. Please do not submit medical records or protected health information through this public form.

Referral workflow
- Referral inquiry received. A hospital, social worker, case manager, placement agency or family member submits an initial inquiry.
- Coordinator review. Our team reviews the inquiry and reaches out to discuss the referral.
- Secure records request (if needed). If sensitive records are required, they are requested and handled through a secure channel — never the public inquiry form.
- Assessment & additional information. Additional information or an assessment is gathered as needed.
- Tour or placement discussion. A tour or placement conversation is scheduled with the family or referring party.
- Admission decision & coordination. Joy in Caring coordinates next steps toward admission.
